Initial Diagnostic Evaluation / Comprehensive Evaluation
Initial Diagnostic Evaluation / Comprehensive Evaluation $450.00: This 120 minute evaluation includes the gathering and reviewing of collateral information from doctors, therapists, and labs, if provided.
Pharmacologic Management
Pharmacologic Management (25 minutes) $160.00; or $310 (50 minutes).
Regularly Scheduled Therapy
Regularly scheduled (weekly or monthly) 50 minute therapy services are seen at a reduced rate of $250.00 per 50 minute appointment. These sessions can be conducted in-person or virtually. Bryan has also been trained in Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT) and in Cognitive Behavioral Therapy for Insomnia (CBT-I) modalities in order to meet the needs of his clients.
Legal Services
Any legal services, including court appearance/deposition $500.00 per hour or Increment.
Forms and Letters
Forms/Letters $25-50: Letters, forms (school issues are exempt), medical records will be charged appropriately, depending on the complexity. Contact us for details.
Prior Authorizations for Medications
Prior Authorizations for Medications for the initial/annual renewal are at no charge. Appeals may be charged appropriately.
Missed Visits and Cancellations
NEW PATIENTS: are asked to cancel their first appointment not less than 3 business days before their scheduled appointment by speaking with me or someone in my office directly. Late cancellation of your appointment will result in a $160 fee. EXISTING PATIENTS: patients are asked to cancel at least 24 hours in advance of the scheduled appointment time. There will be a $50 late cancel charge. The charge for not showing for an appointment is $160. This charge is not payable by insurance and understand that this will be your responsibility.
Form Completion
We will complete medical forms as deemed appropriate at your request for $25 per form. This charge is payable in advance when the forms are submitted for completion. Allow ten working days to complete.
Telephone Calls
For established patients, phone calls between visits are charged at the following rates:
0-10 minutes - no charge
11-15 minutes - $60
16-30 minutes - $125
31-60 minutes- $275
